Community First Choice (CFC) is a Maryland Medicaid home and community-based services program that helps eligible children and adults receive personal assistance and support services in their homes and communities rather than in an institutional setting. Unlike many Medicaid waiver programs that may have enrollment caps, waiting lists, or limited openings, Community First Choice is a Medicaid State Plan benefit available to eligible individuals who meet the program's requirements. CFC is often a good option for individuals who need ongoing personal assistance services and wish to remain safely in their homes while maintaining as much independence as possible.
If you are applying for yourself or helping a loved one explore long-term care options, it is important to understand that Maryland offers several home and community-based care programs designed to serve different populations and needs. Some programs focus on older adults, others on individuals with physical disabilities, developmental disabilities, or medically complex needs. Community First Choice is unique because it provides personal assistance services to eligible individuals who meet a nursing facility level of care and, unlike certain waiver programs, is generally not subject to a waiting list for eligible participants.
Community First Choice may provide support with bathing, dressing, mobility, toileting, meal preparation, housekeeping, shopping, and other activities necessary to remain safely at home.
Depending on the service delivery model selected and applicable program requirements, participants may have the option to choose certain family members, friends, or other trusted individuals to provide paid personal assistance services. This can allow individuals to receive care from people they know while supporting their ability to remain in the community

Individuals providing personal assistance services must meet applicable program requirements, which may include criminal background screening, training, CPR certification, and other qualifications established by Maryland Medicaid and participating provider agencies.
Eligibility is based on Medicaid enrollment, financial criteria, functional need, and a determination that the applicant meets the required nursing facility level of care. Applying is free, and approval is based on an individualized assessment of each applicant's circumstances and needs.
Families are encouraged to learn about available home and community-based programs and determine which option best supports their goals, health needs, and independence. Before applying, applicants should review the eligibility requirements for each program, including age, disability, income, medical needs, and any applicable enrollment limitations.
